Download FormsFreeSupportGroups.com offers free listings to free support groups in the Portland area. We encourage anyone who has a free support group to let us know. If you would like to be listed, you will need to fill out the forms below. You can either print them both and fax or mail them back, or email the Listing Info Form Email Version and print and mail the Listing Agreement. If you have more than one group offering, we encourage you to use the email version of the Listing Info Form and just fill out the portions of that form that are not redundant. Please make sure to include both pages of the listing agreement when mailing or faxing the signed copy. Please keep a copy of any materials or emails sent or faxed to FreeSupportGroups.com until you receive confirmation that we have received it, in case it needs to be re-sent.Thank you.
